If your state has a modern Medicaid app or online account—some do, most don’t—log in. Under “My Providers” or “Claims History,” you’ll often find the provider numbers associated with your past visits.
It’s like a digital breadcrumb trail. See that old appointment from last year? Click it, and the provider number should be listed next to the clinic name. Digital forensics without the FBI badge.
